Case Conferencing
Hosted by the Anchorage Coalition to End Homelessness, these meetings provide a platform for sharing vital information about client circumstances, whereabouts, challenges and assets.
Case conferencing is designed for case managers and those overseeing case management, but outreach and shelter staff from all organizations are also strongly encouraged to participate.

Highly Vulnerable Adult Case Conferencing
This case conferencing is designed to overview medically vulnerable clients within the Coordinated Entry system through both a housing and health care lens.

Family Case Conferencing
This case conferencing is designed to go over housing referrals for households with minors in their custody or who plan to have their children move in with them once they are safely housed.

TAY Case Conferencing
The Transitionally Aged Youth (TAY) Case Conferencing deals specifically with clients ages 18-24 years old who currently have housing referrals.

Adult Case Conferencing
This case conferencing is focused on single adults ages 25+ who currently have open referrals into housing programs.

Veteran’s Case Conferencing
This case conferencing is designed to overview veterans experiencing homelessness, determine eligibility for HUD-VASH and SSVF services, and review clients’ progress on their paths to permanent housing.

Housing Retention Case Conferencing
This case conferencing is focused on clients housed via Coordinated Entry referrals or who are in a Continuum of Care-funded program.
